Each interested in a healthcare career will join an interprofessional team of students. Participants will work with patients to identify issues associated with social determinants of health and propose potential solutions that will aim to reduce hospitalizations and emergency room visits.
Who can participate?
This fellowship is open to students who are seeking practicum placements located in the Hudson Valley region. Applicants who are entering their BSW junior year or MSW generalist (first-year) placement in the fall semester must be willing to commit to participation for two full academic years (in two different placement sites).
Requirements:
Each AHEC scholar will be required to complete an online curriculum, totaling 40 hours each academic year, which addresses topics designed to augment patient care鈥攊ncluding cultural competency, practice transformation, behavioral health integration and interprofessional care coordination.
